Making a complaint outside PACS
We encourage you to make your complaint directly with us but if you feel you would like to take your complaint outside our organisation, please use the following link:
Are you a GP, Medical Practitioner, Nursing or Allied Health Professional?
Do you have a DVA Patient and seeking Community Nursing Services?
We are a DVA Approved provider for Community Nursing Services and can provide services to DVA Gold Card Holders.
Download and complete our referral form and send it to our DVA Coordinators. Once received we will get in touch with the Gold Card holder and begin the assessment process.
- If you have completed a DVA D904 Request Form you can simply send it to our DVA Coordinators instead.
