Partnerships
within the Community.
At PACS, we believe the best outcomes for older Australians come from strong, respectful partnerships.
For over 30 years, we have worked alongside approved providers, community organisations, health professionals and government programs to deliver high-quality care in the home. Our role is simple — to provide reliable, professional services that allow people to remain living safely and independently in their own homes.
We understand that collaboration across the sector is essential. By working together, we ensure clients receive the right services, at the right time, from people who genuinely care.
Our long-standing Partnership with GoCo
One of PACS’ longest and most valued partnerships is with GOCO (Gunnedah Shire Council Home and Community Care Organisation). For more than 35 years, PACS has worked alongside GOCO to support older people across the New England and North West regions of NSW to remain living safely and independently in their own homes.
Through this partnership, PACS delivers in-home services to GOCO’s clients under their aged care programs, including their Support at Home clients. Our role is to provide reliable, high-quality support that allows clients to maintain their independence while continuing to live in the communities they know and value.
Over the decades, this collaboration has been built on trust, strong communication and a shared commitment to delivering consistent and dependable care for older people across the region.
Our Service Regions
PACS currently supports clients and partner organisations across:
- New England NSW
- Central Coast NSW
- Darling Downs QLD
Our regional model allows us to provide consistent, responsive services to communities that are often harder to reach.
Our Partnership with Trilogy Care
PACS is proud to partner with Trilogy Care to support older Australians who choose to self-manage their Home Care Package.
Through this partnership, PACS delivers in-home services to Trilogy Care clients across our service regions, providing reliable and professional support that enables people to continue living safely and independently in their own homes.
Working with Trilogy Care allows clients greater choice and control over their care, while still having access to experienced local service providers. Our team works closely with clients and Trilogy Care to ensure services are delivered consistently, respectfully and in line with each client’s individual needs and preferences.
Our Partnership with Carer Gateway
PACS works in partnership with Carer Gateway to support carers within our communities.
Through this partnership, PACS provides services to carers who may be experiencing strain, helping them access practical support and respite when it is needed most. Our team works with Carer Gateway to ensure carers and the people they care for receive reliable, compassionate assistance that helps them continue living safely at home.
By supporting carers through this program, PACS contributes to strengthening the wellbeing of both carers and the individuals they support within our regional communities.
Our Partnership with the Transition Care Program (TAP)
PACS works in partnership with health services through the Transition Care Program (TAP) to support older people returning home after a hospital stay.
Through this program, PACS provides in-home support services that assist clients during the transition period following discharge from hospital. Our team works closely with healthcare providers to ensure clients receive the practical assistance they need while they regain confidence, independence and stability at home.
This collaboration helps reduce hospital readmissions and supports older people to safely transition back into their own homes and communities.
